Treatment begins with an accurate and precise diagnosis. At Mt. Vernon Spine and Disc Center, we use the most advanced methods combined with a comprehensive examination in order to precisely diagnose and then treat your condition. 

In preparation for your first appointment, there are a few things you can do to help us. Please collect any pertinent medical records, including x-rays and MRIs. If you have had imaging studies, please try to bring the actual films and not just the reports. These will be helpful for us to review. Please also download and print the following form and complete it prior to your visit. Alternatively, plan to come fifteen minutes prior to your appointment, at which time you can complete any additional paperwork in our office.
